Grant Recipients 2012-2013

The Southeastern Colorado Affiliate of Susan G. Komen for the Cure is proud to distribute grants totaling $270,214 to six worthy organizations providing vital services in El Paso, Pueblo, and Teller Counties. Sense of Security
Financial Assistance for Breast Cancer Patients
This program provides monthly financial assistance to breast cancer patients who are undergoing treatment. Patients receive up to $500 in assistance each month, for up to six months, while they are in treatment.

Homeless Women’s Breast Health Program
This local program focuses exclusively on the breast health of homeless women. A case management approach will be used to educate this population regarding the benefits of early detection.

This grant provides breast cancer diagnostic services and case management for women who do not have other medical coverage. It insures individuals have sustained medical access while guiding them through the continuum of care.

Breast Cancer Continuum of Care
This program will provide mammograms to women who lack health insurance or who have rarely or never been screened. A Genetic Counselor provides education, assessment, and counseling to high-risk women or those already in treatment for breast cancer. A Patient Navigator provides education and follow-up care to women who have completed their treatment.

Women’s Breast Health
This grant will provide screening and diagnostic services for qualified women as well as fuel vouchers to allow access to screening facilities. A nurse case manager will provide health education and follow up of services to women enrolled in this program.

SMALL GRANT

Tapestry of Healing Support Group
This group was created for women with advanced and metastatic breast cancer. The group meets MONTHLY on the 2nd Thursday of the month from 4:00 to 5:30 p.m. at the American Cancer Society located at 1445 N. Union Blvd., Colorado Springs, CO 80909. Komen) to bring education, support and resources to women with metastatic breast cancer.

Circle of Hope Patient Financial Support Program

The H.O.P.E. Retreat
The H.O.P.E. Retreat is a four-day retreat designed by the late Cathy Schulze, a woman who lost her life after a long battle with breast cancer. The retreat combines education, group facilitation and personal time for self- reflection, exploration and integration of each woman’s cancer journey.

2011 Colorado Breast Cancer Resources Directory
The grant will be used to support cost for publishing and distributing the Colorado Breast Cancer Resources Directory as well as support for maintaining the web version of the guide.  This is a helpful guide to the broad range of resources statewide for patients, survivors, caregivers, and the healthcare community and has information that addresses the continuum of care from screening to survivorship.

Penrose Cancer Center
Multidisciplinary High Risk Breast Clinic
Patients will consult with several breast health care specialists in a single visit to develop a risk management plan.  Grant-funded imaging studies will be available to uninsured and under-insured high risk women.  The Clinic will expand existing medical oncology, breast health navigation, and hereditary cancer services to widen the continuum of care to unaffected but high-risk individuals under age 50.

This program closely monitors patients with personalized, comprehensive case management services.  Staff will educate patients about the importance of early detection and encourage screenings and other preventive health care measures.  Grant funds will provide for breast cancer ultrasound screening, surgical consultations and other necessary diagnostic services for high-risk women under the age of 40, as well as men and women over the age of 40 who do not have other coverage options.
